Participation in any services through the Company includes, but is not limited to, education, fitness, health services and on-site educational workshops. I understand that in a group setting, I am in control of if, and when, I leave the setting. If at any point I feel overwhelmed or uncomfortable, I will respect my own body and mind’s limitations and I will leave the premises and notify the teacher or instructor or Company as appropriate.

I understand and acknowledge that in any event there exist certain inherent risks in participating. I voluntarily participate in the services provided by the Company and agree to assume full responsibility for all risks, injuries, or damages known or unknown, which might incur as a result of participating in the services.

I understand that it is my responsibility to consult with my health care practitioner if applicable prior to and regarding my participation in services offered at Company and I have consulted a medical doctor or licensed medical heath care practitioner or had the opportunity to do so before participating.

I acknowledge that all fitness and health activities entail known and unanticipated risks which could result in physical or emotional injury, paralysis, death, or damage to me, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity. The risks include, among other things: my own physical conditions or my own acts or omissions. Furthermore, the Company seeks safety, but they are not infallible. They might be unaware of a participant’s fitness or abilities. They may give inadequate warnings or instructions, and the equipment being used might malfunction.

I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless the Company from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of the Company equipment or facilities, including any such claims which allege negligent acts or omissions of the Company. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I have no medical or physical conditions, which could interfere with my safety in this activity, or else I am willing to assume – and bear the costs of – all risks that may be created, directly or indirectly, by any such condition. The premises where activities and services are held is inherently dangerous and may result in injury or death.

Privacy is not guaranteed while delivering services if it is a group activity or services are provided on-site at a third-party location such as nature or a park or outdoors.

By signing below, I release Company, its owners and instructors, event facilities, event partners, as well as other participants from liability, and hold them harmless for any injury to my person, and damage to my property while receiving services with Company, whether caused in or out of the services provided, by negligence or otherwise.

I understand that Company may provide an area for personal belongings to be held during services provided, however, I agree that Company is in no way responsible for the loss or damage of my belongings while I receive services.

I acknowledge and accept that all fees, once paid, are not refundable unless other payment policies are offered in writing in conjunction with this Agreement.

IF UNDER 18 YEARS OF AGE: As legal guardian of the participant, we consent to the above conditions.
